Question: My passport was printed with an incorrect space in my last name, i.e., Mc Coll instead of McColl. Will this be a problem for use of the passport? (It does not match my other identification documents because of this space).

Answer: Errors in spaces do not usually cause any issues during travel. For good measure, you may book tickets with a space in your last name to match your passport and totally avoid issues with TSA.

Dash between last names

Question: I have two last names and no dash in between. The passport office decided to put a dash. But that is not my legal name. It appears with no dash on my Social Security Card and driver's license and employer. How do I tell you not to place a dash between last names when renewing my passport?

Answer: You may submit your passport for a correction at no cost to you. Please mail the following to the address on the form:

1. Form DS-5504
2. Passport with error
3. Proof of the correct data
4. Signed statement explaining the error
5. One new passport photo
6. Check for $60 if you want expedited service

Missing middle name on passport

Question: My name on passport missing middle name while documents for the application e.g. ID and naturalization cert. have my full name. I cannot apply for a visa to Vietnam with this mismatch. So can I use form DS 5504 to fix it?

Answer: Since it is not an error on the passport, you cannot use DS-5504. Please use DS-11 and submit a new application in person.

My daughter's middle name has one letter wrong

Will my passport number change?

Question: I recently my passport back and noticed that my last name has an extra S in it. UGH!! I need to get a working visa soon and the hiring personnel is asking for my passport number. Can I give them the passport number on this book before sending my passport to be corrected, or will my passport number change when I get a new updated book?

Answer: You will be issued a new passport with a new number. Corrections are processed as a free expedited service so you can apply at a regional passport agency and get your new passport in 1 to 8 business days.

You need to secure an appointment to apply at an agency but if none is available, you may walk-in with your application. Just make sure to arrive before the agency opens to increase your chances of securing a slot.

Incorrect surname on new passport

Question: Hello , I received my passport but there is an error in spelling of my last name. I read somewhere that if it's within 90 days I should send it to the agency that processed it (am guessing it's the return address on the envelope it was mailed in.

Answer: Please send Form DS-5504 and other requirements for a correction to the agency that processed your application. The address is printed on the envelope your passport came in.

Will a typo regarding a hyphen in the name cause problems?

Question: My son's passport was renewed and the new one was printed with a double hyphen in his middle name, instead of a single hyphen.
Will this cause problems?

Answer: A minor typographical error such as this should not cause problems.
