Fort Collins Passport Offices

The passport application form DS-11 can be submitted along with supporting documents to an authorized agent at the nearest Fort Collins passport office. The agent will review the documents, administer an oath and witness the signing of your application. Applications for passport renewals, name changes and the corrections of typing errors can be submitted by mail. For Urgent Travel Service, you need to call 14 days before your scheduled international travel or 28 days if you also need to secure a foreign visa.
