Marital Status Questions on Passport Application

Question: It bothers me that I am being asked about my ex husband. The question states, "Have you ever been married?" but then goes on to ask for his name, birth date, place of birth, and citizenship status.

The application will not allow me to continue without answering these questions. My question is what will happen if I simply answer NO to "have you ever been married?".

Answer: You can print a copy of the application form and complete it in ink. The application form DS-11 has this statement concerning requested information. Click the link below to download a copy.

Form DS-11: Application for a U.S. Passport

"CONSEQUENCES OF FAILURE TO PROVIDE INFORMATION: With the exception of your Social Security Number (see Federal Tax Law
statement on Instruction Page 3), you are not legally required to provide the information requested on this form. However, failure to do so
may result in Passport Services’ refusal to accept your application or result in the denial of a U.S. passport."
