Trying to leave Guam

Question: I am trying to leave Guam and I have two kids and their father has a stay away order. Could I still get their passport without his consent?

Answer: If the restraining order is still in effect, you may submit it with Form DS-5525 and the rest of the application requirements for a child's passport.

Please note that applications sent with DS-5525 are processed on a case by case basis. Your children may or may not be issued passports. Please call 1-877-487-2778 for more information.
