Form DS 82 Step 10 Most Recent Passport Book or Passport Card

Question: On DS-82 form, step #10 asks for name and date issued. The third space says, "Most recent passport book or passport card." What info do we need to fill in this space? I have entered my full name and the date issued in the other 2 blocks.

Answer: *Application form DS-82 is used to renew a passport. One of the requirements is that you possess a passport that was issued within the last 15 years that is not significantly damaged and you were not a minor when it was issued to you. Step 10 of form DS-82 is where you enter data concerning your previously issued passport.

There are three parts to this step. One is "Your name as listed on your most recent passport or passport card." Another is "Issue date." The last part states "Most recent passport book or passport card."

What this is asking for is your passport number. In a passport book, this can be found at the top of the personal information page. The number of the passport card can be found on both the front and the back of the card.
