Prior names don't fit in box

Question: In section 9, it says to list all other names you have used - neither of my previous names fit in the boxes provided, they are both too long. How do I enter this information? Do I simply write them on a separate piece of paper and include them with my application, indicating that these are my previously used names? Do I enter anything into the section 9 boxes, such as see other page?

Answer: You may attach a separate piece of paper listing other names you have used in the past. Label the paper as such. There is no need to enter anything in the Section 9 boxes.

Question: In section 9, it says "list all other names you have used". Do I write down my literal maiden name, or the words "Maiden Name"?

Answer: Please write your maiden name. For example: Carolyn (Maiden Last Name).
