Section 10 of Passport Application: Parent Names
Question: When I filled out the application using the online form, the Date and Place of Birth for parents was not listed as required fields.
Are these fields required to be filled out for an adult applying for a passport for the first time?
Answer: The online application form and the printable passport application are slightly different. You are only required to provide the information requested on the form that you choose to complete.
