I am debating on waiting to change until it actually expires if it does not renew with the name change form and fees. My current passport expires next year but it has my Maiden name. Answer: The form you will use to change the name on your passport is Form DS-82 which is actually the renewal form. You will be issued a new passport that will be valid through 2029 if you apply for the renewal with name change now.
