Question: What do I need to do if I just realized that the birth year on my passport is incorrect and does not match my American citizenship certificate? At this time I do not need to renew my passport. I plan on flying out of the country in February next year and realized that the year printed on my passport is incorrect.
I provided the office with my American citizen certificate which has the correct birth year when I first applied for the passport. I traveled with it before and had no trouble coming in and out of the country but I am worried that I might one day and don't want to take that chance.
Answer: You can request a
passport correction for an incorrect date of birth by submitting
form DS-5504 to the address on the form.
You can do this at any time during the validity of the passport with additional cost.
It will be necessary to submit your present passport and evidence of the correct date of birth.
