Middle name on passport, no middle name on birth certificate by: Joan M Question: My birth certificate does not have a middle name. However, I added a middle name at the time of my confirmation and I use my middle name on both my driver's license and my passport. I now want my driver's license and my passport to be consistent with my birth certificate. What form do I need to complete and what do I need to provide to drop my middle name on my passport? Answer: The best way to do this is to change the name on your license first. Once that is done, you may submit a new passport application. The passport will now be issued in the name you enter on the form since it will be supported by your birth certificate and ID (license).
