How do I see the name that will be listed on my passport?

by Angela
(Granger, Indiana)

Question: My passport has been applied for but I need to get my airline tickets. Will the passport have my full middle name or just the initial?

Answer: Your passport will have your full name in it, including your complete middle name.

The US Transportation Security Administration (TSA) requires all passengers to enter their full name – as it appears on the government issued identification they will be traveling with – when making airline reservations for international flights. They also require date of birth and gender.

TSA officers at airports across the country will check to make sure they match when passengers pass through security. According to TSA, Secure Flight will be phased-in with each airline. Passengers should not be concerned if particular airlines do not ask them to provide the additional information right away; it should not impact their travel.

Opposite name on passport
by: Tracy

Question: My birth certificate has first name+last. In my passport, the given name field has my last name and the surname field has my first name. Is this okay?

Answer: It is not ideal but you may use that passport. Just make sure that you book your tickets in the same name that appears on your passport.

If you want to have the passport corrected, you may do so by filling out Form DS-5504 and submitting it with the following:

1. The passport with the error.
2. A signed statement explaining the error
3. Evidence of the correct data (birth certificate)
4. New passport photo if it has been over 90 days since the issuance date

There is no fee for this service unless you want it expedited, in which case you have to pay $60. Please send the required documents to the address on the form.
