Form to use to renew 16 and 17-year-old children's passports.
Question: What form do I need to renew my 16-year-old and 17-year-old children's passports? Do both parents need to be present when obtaining the passport? Can I do it by mail?
Answer: Passports that were issued to minors cannot be renewed by mail. Your children must apply for new passports using
Form DS-11.
All Minors Age 16 and 17 shall:
* Appear in person at the time of application;
* Have a parent or guardian present photo identification if the minor does not have identification of his or her own;
* Provide a photocopy of the same ID document that will be presented at the time of application (applicant or parent/guardian);
* Establish parental consent (You may be requested to provide written parental consent for a child age 16 or 17 if it is not implied on the application).
At least one parent needs to be present for a 16-year-old to apply. Usually, a minor age 17 can apply without the presence of a parent.

If you need to get the passports within 2-3 weeks, you can make a request for expedited service at the time you submit your application.
If you need to get them in less than two weeks, you will need to either submit the applications at a
regional passport agency or apply through a registered
passport expediting service
