Answer: The success of applications submitted with Form DS-60 Affidavit Regarding a Name Change depends largely on the documents that you submit with it. DS-60 must be executed by at least two persons (preferably blood relatives) who have known the you by both names and that the new name has been used for at least 5 years. In addition to the affidavits, you must submit at least 2 or more public documents evidencing that the you have used the acquired name generally for five years or longer.
