Question: If I was adopted, do I need to put my old name on the passport application?
Answer: The name that you put on your passport application should be the same name that is found on your evidence of U.S. citizenship (usually a certified birth certificate) and proof of identification (usually a valid driver's license) at the time you submit your application.
