Question: I applied for a passport. I gave BC from courthouse and state issued ID. Asking for 5 more documents I gave a copy of my diploma dated, a check stub from a old workplace, a old credit card bill, a old photo badge from previous job and a copy of my baptism certificate and SS card. I hope that will work because the post office said it may not.

Legal Name Change

Question: I had a recent legal name change. I submitted my amended birth certificate and drivers license with my new name. The post office clerk said will be fine yet was rejected. I obviously do not have documents with my new name that are older than 5 years. Will the original of the court order for the name change be sufficient?

Answer: You need to prove that the person whose name appears in the citizenship evidence, ID and application form is one and the same. If you can provide proof of the name change like the original or certified copy of your marriage certificate, it should be accepted.

Question: Hello, I recently applied for the passport but I received a letter for additional identification purposes. What other ways could I prove my identity besides the listed ones on the letter? Could I use my car registration, insurance card, health card, bank debit, social security, house rental contract, high school diploma, high school yearbook? I had recently booked a flight thinking i wasn't going to have any problems, Would i be able to expedite this service?

If you are traveling in 2 weeks or less, you may expedite at the closest regional passport agency. You need to present your airline tickets as proof of immediate international travel in order to be entertained.

Super scared my passport won’t come in time

Question: I earned an amazing trip with my company. I got a letter saying I needed To send in 5 additional proofs. I am still waiting on a call but a lady at the regional agency said what I had should be good. In Iowa, we don’t sign our children’s birth certificates and they throw away all traffic tickets so I was coming up short on my secondary identification records. I ended Up with a huge stack of papers and had my best friend fill out a witness form saying I am who I am. I sent in my itinerary and expedited. The lady at the recorders office who has helped me 5+ times now says they’ll adhere to my travel dates but I’m so scared because everything is saying 8 weeks.

Passport taking forever

Question: I applied for my passport on 11-22-17, I had received an email stating I needed to provide 5 or more ways to identify my self, mind you the email was sent on 12-12-17. Would I still be able to submit the 5 forms or more identification items even though it's been over 90 days? I called U.S. passport services number and they told me my passport application is still in process. I called the passport service on 3-28-18. How long will the 5 forms of ID take to process?

Question: I received a letter stating I need information to prove that I’ve used my assumed name. My birth certificate has my birth name but my name wasn’t changed legally. However, I’ve been using my assumed name for 40 something years. I have a social security printout and bankruptcy discharge paper work and voters registration and Sam's Club all in my assumed name. Will this help? My cruise in June 11th 2018.

Question: Applied for passport 02/01/2018 paid all the fees to ensure everything would be ready before travel date (03/01/2018) . It’s 02/12/2018 and I’m just receiving the email saying I need to submit 5 more pieces of info. Will this process take longer than planned and now I could miss my trip because the passport won’t be ready? I submitted my State ID and original birth certificate. Why wasn’t that enough?

Answer: A state-issued non-driver ID is considered a secondary proof of identity. When submitting a secondary ID, you should include other forms of ID such as your SS card, voter's registration or company ID. This is most likely the reason why you were asked to provide five more proofs of identity.

You need to comply with the additional requirements soon. Once passport services receives your IDs, processing will resume and your passport should still arrive within the 2 to 3 weeks guaranteed by expedited service. Question: Submitted an application for our 3 months old baby girl, got back a letter asking her to send 5 or more documents 5 years or older. To prove her citizenship. They must be out of their minds. Have called the information center several times, no answers, just asked to wait for someone to call back. We are completely stuck and very disappointed of the system. What a joke.

Answer: The following documents are acceptable as evidence of U.S. citizenship:

1. Original or certified copy of your child's U.S. birth certificate (can be obtained from the city/state vital records office where she was born)
2. Consular Record of Birth Abroad (if the child was born overseas, obtain from the U.S. Embassy or Consulate)
3. Naturalization Certificate
4. Certificate of Citizenship

Question: I have gotten a request for the 5 extra forms of ID older than 5 years. I don't know a single 31-year-old with these kind of files. I have diplomas, from US. I have old pay stubs but again, who signs their pay stubs before getting them? Is there any other way to prove my identity other than that list? I don't have those type of files. I've already sent on my birth certificate and copy of my social with my original application. I am an RN but we don't get hard copies of our licenses anymore, it's all online. Same with medical bills, etc. Am I just screwed and apparently not going to be able to travel overseas?

Question: I was issued a passport, needed to renew it, filled out the renewal form, submitted the required documents and received a letter stating that they needed further documentation. Why would they need further documentation when they issued me a passport already, this was only a renewal of the original and the original passport was included? They are asking for school documentation, pictures, school records and so forth, I have no contact with my family. I have nothing from my childhood, how do I get the documents they are requesting so I can get my passport renewed? Now it is past the 15 years, should I just apply for another one?

Answer: If it has been more than 15 years since your passport was issued, you need to apply for a new passport.

As long as you have all the requirements in order, you should have no problems obtaining a new one. You need the following:

1. Form DS-11 (completed but unsigned)
2. Evidence of U.S. citizenship
3. Proof of identity
4. Photocopy of ID
5. One passport photo
6. Payment for fees

Please take your requirements and apply in person at a nearby acceptance facility.

Passport denied

Question: My bf and I went to the post office, they did all the paperwork, we both submitted our original birth certificates and they made copies of our valid drivers license (both instate) nothing at all abnormal, last week he gets his passport, today I get a letter saying my identification I provided was not sufficient! They are requesting 5 things which are 5 years or older, ok so I'm 37 years old. I do not have my learners permit, my drivers license is current, I don't have mine from 5 years ago, never had a passport before and I have no newspaper or magazine articles of me from over 5 years ago. The next list I have none of nor have I ever had : employment ID card, professional license, federal or state ID card, military ID, student ID (ok I went to school back in the 90's, really)
The third list should have legal signature and date :
Military records: no
Selective service: no
Union membership: no
Health insurance card: not from 5 years ago and certainly not with a signature
Marriage certificate.... I've been divorced 17 years so no I don't still have that
Children's birth certificate with signature: no
Social security card: YES but they don't have dates
Traffic ticket: no
So what do I do???? I tried calling the 800 number they have on the letter if you have questions, and I was informed I needed to read the letter, which I have over and over, I was finally told someone would call me back. What the heck to I do???!

Question: I received a call from a man who said he needed a copy of my student ID even though they have my birth certificate and drivers license. He said it was because students usually don't have a lot of history. When I asked if he looked at my documents to see that I am in my 30s, he said do you want a passport or not... Then I asked what constitutes a lot of history? He said that he could just suspend my application or I can email him the ID. This seems a little odd. Is this customary and what does he mean not a lot of history?

Answer: Your birth certificate serves as your evidence of citizenship while your driver's license serves as your proof of identity. Unless you submitted an out-of-state driver's license, you do not need to submit additional identification documents.

Why send documents that are five years old?

Question: This whole process is ridiculous. I have submitted everything that was asked of me and still have to submit five documents that are five years or older. The number that was given to me from that agency doesn't help. The only thing they did was take note to submit a request for someone to call me. I just wanted to know what if I do not have documents that are not five years or older. What other pieces of information is needed. No one can seem to tell me. And what ever answer I get I know will be bull crap. We claim to be one of the greatest countries but can't look into someone citizenship with just a social security number, license and birth certificate. Computer hackers can tap into your stuff with way less info. So sad.

Answer: Applicants who cannot submit primary evidence of citizenship need to provide secondary proof. A birth certificate can only be accepted as primary evidence of citizenship if it is in long form (issued by the city, county or state). The original or a certified copy (has a registrar's raised, embossed, impressed or multicolored seal, registrar's signature, and the date the certificate was filed with the registrar's office) must be submitted.

Secondary proof of citizenship includes early public records, a delayed birth certificate, and a birth affidavit, among others. Early public records are documents ideally created in the first five years of your life like a baptismal certificate, hospital birth certificate or census record. You must submit early public records with a birth record or Letter of No Record.

These documents are required to establish your eligibility to obtain a U.S. passport.

Passport Agency Asking for More Documents

Question: The Passport Agency contacted me to request a Statement for all my family's DOB, Place of Birth, Citizenship, all previous residences, schools attended and work history.

I am currently in the process to get my 1st passport. I am in need of it because of religious purposes. I recently called the number for the agency and they told me it was being processed. I usually call every week to check the status of the application and they had never informed me about this requirement.

This is the 6th time I applied for a passport. I even sent information that they requested back in March and now they are asking me to send a "statement" with all the info that I already mentioned. I have called so many times and they told me that the Charleston agency would contact me if they need more info but they never did! Also, the lady stated that I have a foreign birth certificate and that I need to give an explanation about this. This is totally not true. As for my knowledge, I have never obtained a foreign birth certificate. This is really sad. It is our right as American citizens to obtain this document!

Answer: Passport application form DS-11 states, "You must establish your citizenship to the satisfaction of the acceptance agent and Passport Services. We may ask you to provide
additional evidence to establish your claim to U.S. citizenship." It goes on to states, "You must establish your identity to the satisfaction of the acceptance agent and Passport Services. We may ask you to provide additional evidence to establish your identity." We recommend you provide the documents requested.

5 TO 10 YRS BACK!!!

Question: I applied for my 1st passport to be expedited. Brought original documentation amd paid $240. I was told 2 to 3 weeks in order to receive my passport, but all I got were emails requesting much more than the original 3 pieces. I was born in the USA with no criminal record so why the heck am I being treated otherwise?!!! It's pretty much false advertising to state bring 3 pieces of original docs when you want a life time of them. I was asked for ridiculous things dating 5yrs back which I didn't have much of and then another emial asking for 10yrs back. If I did not have all of the 5yrs then why the heck do you expect me to have 10?!!! I wrote my congressman and the better business bureau to see if this is a scam of some sort. This entire process is a nightmare and no one can tell you why you are being treated like less than.

The instructions section of passport application form DS-11 states the following.

You must establish your citizenship to the satisfaction of the acceptance agent and Passport Services. We may ask you to provide
additional evidence to establish your claim to U.S. citizenship.

It goes on to state the following.

You must establish your identity to the satisfaction of the acceptance agent and Passport Services. We may ask you to provide additional evidence to establish your identity.

For some reason, the agent processing your application did not believe the the documents you provided established your citizenship and/or identity and, therefore, requested more.

You can call 1-877-487-2778 to speak with a customer service representative but they usually cannot provide details of the process.

Question: So essentially, it's completely arbitrary and the passport agency just does whatever it pleases. I am in a similar position. I'm 37 and filed to get my first passport. Had all my information and now the passport agency sent me information asking for at least 5 pieces of documentation at least 5 years old that has pictures, signatures, etc on it.. Why? Apparently the answerer here will not give any information other than "If they ask you for it you have to give it, and they don't have to give any reason whatsoever." This is infuriating. What documents which are "at least 5 years old" that have my photo and signature on them am I going to keep lying around? Maybe I have one or two, but FIVE? It's arbitrary and it's wrong. I guess I should feel lucky they didn't ask me for documents longer than 20 years. Talk about ridiculous. Guess he angered the wrong person or the wrong agent had a bad day.

Answer: Additional documents are usually requested when the ones submitted do not meet the criteria established by the Department of State. For example, the birth certificate must be an original or certified copy issued by the City, County, or State of birth that lists bearer's full name, date of birth, place of birth and parents full names. It must have been registered within one year of birth and have the registrar's signature and an embossed, impressed, or multicolored seal of registrar. There are other criteria for the identification document.

If the name you requested for your passport does not sufficiently match the citizenship and identification documents you submitted, then you must establish that you have used the name for at least 5 years.
