Question: I'm planning to apply for a US passport for my new baby. Is it possible for me to apply as a single mom? Since I got pregnant, the father of my baby disappeared. I don't know where he is.
Answer: A birth certificate without the father's name on it can be submitted as evidence of sole legal custody. If the father's name is on the birth certificate, you will need to either submit notarized consent from the father or Form DS-5525: Statement of Exigent/Special Family Circumstances.
The statement must explain in detail the non-applying parent's or guardian's unavailability and recent efforts made to contact the non-applying parent.
