We are guardians for a 4-year-old and will be traveling overseas this summer. What documentation is needed to get his passport?

All minors must apply in person accompanied by both parents or guardians. It is necessary to submit a completed application form DS-11 and proof of your ward's U.S. citizenship.

You must provide evidence of your relationship to the child such as a court order of custody or guardianship. Both guardian(s) must present acceptable identification at the time of application and provide photocopies of the identification documents.

If one of the guardians cannot be present at the time of application, the applying guardian must submit a notarized consent form from the non-applying guardian or evidence of sole guardianship.
