Section 10 of Passport Application: Parent Names

Question: When I filled out the application using the online form the Date and Place of Birth for parents was not listed as a required field.

Are these fields required to be filled out for an adult applying for a passport for the first time.

Answer: The online application form and the printable passport application are slightly different. You are only required to provide the information requested on the form that you choose to complete.
