Question: Hi, I want to apply for an American passport for my kids of 4 and 5 years, but they don't have any SSN what do I enter on the DS-11 form when it asks me for the SSN?
Answer: Providing the SSN is voluntary but lacking an SSN in the application may cause delays in processing. The SSN is used to verify the identity of a passport applicant. If you insist on applying for the passports without SSNs, you may write zeros in the spaces and include a signed statement with the following:
"I declare under penalty of perjury under the laws of the United States of America that the following is true and correct: (Child's full name) has never been issued a Social Security Number by the Social Security Administration."

Lost social security card by: Anonymous
Question: I applied back in May 2016 for my children's passports. I received the passport and the passport card but I just noticed I NEVER received their social security cards in the mail. How may I obtain them?
Answer: Supporting documents are mailed back to applicants. If you have not received them by now, they were probably lost in the mail. You can call 1-877-487-2778 to speak with a customer service representative. You will probably need to apply for new SS cards. You can get more information on the website of the Social Security Administration.
