passport renewal and name change

I need to renew my passport as well as change my name (married). Do I need to submit both DS11 AND DS82 form?

If you meet all the criteria for renewing a passport by mail, then you only need to submit form DS-82.

Besides the application, you need to submit your most recent passport and two passport photos. Due to your name change, you need to submit proof of this change.
