Passport Name Change

Question: I need to change my last name on my passport from my married name back to my maiden name. Once my divorce was final, I took the proper paperwork to the social security office to change my name & drivers license.

What do I have to submit to show my name has been changed?

What is the change fee & form I need to fill out?
I currently have a passport, but it is issued in my married name. I am over 16 years old & the passport is only about 5 years old.

Please advise. I need to send my paperwork for travel in October.

Answer: Since your passport was issued more than 1 year ago, you must apply for a passport renewal. You need to submit the following.

1. Completed passport application form DS-82
2. Two passport photos
3. Divorce decree (The decree must state the fact that your name has been changed and give the new name.)
4. Passport Renewal Fee
