How do I get a passport for a child born to English mother and American father?

Please, could you provide me with information on obtaining a US passport for my child, her father has legally signed court papers in CA as her legal parent but is not yet on her British birth certificate, which I am currently looking into in Oxford UK. Your help would be much appreciated on this matter. You must provide acceptable evidence of U.S. citizenship. You must also provide either a notarized consent form from the father or legal evidence of sole custody. The following documents serve as primary evidence of citizenship: * Previously issued, undamaged U.S. Passport * Certified birth certificate issued by the city, county or state * Consular Report of Birth Abroad or Certification of Birth * Naturalization Certificate * Certificate of Citizenship If none of these is available, you can submit secondary evidence of citizenship.
