Question: I have an appointment at the Connecticut Passport agency to obtain a new passport. I am replacing a lost passport given out in July, 2008. It was never used and was inadvertently thrown away at my house. Do I need to bring something to the appointment? I don't have any information on the old one.
Answer: Since you cannot present your most recent passport, you will have to apply for a new one using form DS-11. This also requires you to submit the following documents:
* evidence of U.S. citizenship (e.g., certified birth certificate)
* proof of identity (e.g., valid driver's license)
* photocopy of identity document
* two passport photos
Note, you can complete the application form before arriving for your appointment but should wait to sign it until requested to do so by the passport agent.
You also need to take your Social Security card and appointment confirmation number.
You are encouraged to report the lost passport by calling 1-877-487-2778 or completing form DS-64.
