Answer: Here is a list of items you will need to apply for a new passport.
1. Completed but unsigned application form DS-11.
2. Evidence of Citizenship. Your options are...
a. Previously issued, undamaged U.S. Passport
b. Certified birth certificate issued by the city, county or state*
c. Consular Report of Birth Abroad or Certification of Birth
d. Naturalization Certificate
e. Certificate of Citizenship
3. Proof of Identity. Your options are...
a. Previously issued, undamaged U.S. passport
b. Naturalization Certificate
c. Valid Driver's License
d. Current Government ID (city, state or federal)
e. Current Military ID (military and dependents)
4. Photocopy of the Identification Document. The copy must be...
a. on plain white, 8 1/2 x 11" standard paper stock, showing the front and the back of your ID
b. contain images on only one-side of each page submitted.
c. free of other images and/or markings.
5. Two passport photos. The pics must be...
a. Identical
b. In color
c. 2 x 2 inches in size
d. Taken within the past 6 months, showing current appearance
e. Full face, front view with a plain white or off-white background
f. Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
g. Taken in normal street attire
6. Pay the fees. They are...
