Do I need to submit proof of citizenship to renew an emergency passport?

Question: My passport was stolen this May while I was in Romania and the US Embassy there issued me an emergency passport that was limited in validity. I have filled out the DS-5504 form in order to renew it but am unsure if I need to mail additional evidence of my citizenship because the passport was valid for less than one year. If so, what form of evidence is needed?

Answer: If your passport was issued under 22 CFR 51.41, you also need to submit proof of citizenship. If your passport was issued under 22 CFR 51.28, you need to submit proof of identity. This information should be in your emergency passport.

Evidence of United States citizenship includes a certified birth certificate, Consular Report of Birth Abroad or Certification of Birth, Naturalization Certificate or Certificate of Citizenship.
